Unique Participant Code (UPC):

The following information is used in each of our questionnaires to generate a code that will ensure the anonymity of your identity and that is unique to you. It will also serve as a field key that allows us to link together your responses to multiple questionnaires. If this is not the first time you have visited the Virtual Dream lab, please ensure that you respond to all of the questions exactly as you did on previous logins so that your code will be identical each time. Note that the fields are not case-sensitive.

Last two letters of your family name: (Example: SMITH)
Second letter of your given name: (Example: JOHN)
First two letters of your city (town, village) of birth: (Example: OTTAWA)
Last digit of your year of birth: (Example: 1968)
Two digits of your date of birth: (Example: MAY 17)

You are 18 years old or over. Your participation is on a voluntary basis and you may withdraw at any time and for any reason with no penalty. You will only be identified by your Unique Participant Code (UPC). All the responses collected from you will be kept strictly confidential and treated in an anonymous manner. Results may appear in a publication or other kind of written document, but no information that can identify you will be disclosed. If you change your mind and wish to withdraw your results from our database, send an email to withdraw@dreamscience.ca that contains your Unique Participant Code.
